What to bring
- Insurance card(s)
- Driver's license or valid photo ID
- A current list of medications
- Referral, if your insurance plan requires one
- Copayment or deductible, if applicable
New-patient forms
Call the office you are visiting and ask for the New Patient Packet by email. Completing it before you arrive shortens check-in.
Payments
We accept Visa, Mastercard, American Express, Discover, and CareCredit. Copays are due at the time of your visit.
Surgery and insurance
Most plans we accept have no deductible for in-office surgery, so patients usually pay only their copay. Outpatient surgery at a hospital or surgery center usually applies your deductible first. Ask us before any procedure and we will walk you through your plan.
